Generally speaking, forts were defended in the US Civil War by placing troops in front of the fort, or fortified from the direction in which the enemy was expected to attack. If these troops were ineffective, then the commanding officer could fall back to the fort to defend it and his troops from the enemy. Perhaps the battle of Fort Donalson and the final Union assault on Vicksburg are excellent examples of this.
