John Adams was defeated for re-election by his VP, Thomas Jefferson in 1800.
That occurred because of the way vice-presidents were chosen in those days. Jefferson became VP because he came in second to Adams in 1796.
Gerald Ford was the only vice-president not elected. He was appointed VP when when the elected VP Agnew resigned resigned.

The VP presides over the US Senate. He can vote only if there is a tie vote.
If the presidency is vacancy. the VP becomes president and and new VP is promptly appointed . If the president and VP both die at once, the Speaker of the House becomes president.
If the VP dies first, the president would choose a new VP. If the president dies first,. the VP would become president and then would choose a new VP. If the president and VP both die at the same time, the Speaker of the House would become president and then he would choose a new VP.

It has always been the law that the Vice President assumes office if the President dies or is incapacitated. This has happened eight times since 1788. There has never been a case where the VP, becoming president, is also killed or incapacitated. The closest thing ever to happen like this was when Nixon was President and Spiro Agnew was VP. Agnew was corrupt, and resigned as VP to avoid impeachment. President Nixon nominated Gerald Ford to be VP, and when Nixon resigned to avoid impeachment, Ford became president. Ford is the only person to be president who was never elected as President or VP.

You would elect a new vp but if it were the president the vp would take his place.
If the President had been a VP themself, and then had been sworn in when the President had died. The VP office would then remain vacant until the next election. This changed under Richard Nixon, so that if a VP became President, a new VP would need to be appointed. As a result, when Nixon resigned, his VP Gerald Ford became President, and Nelson Rockefeller was appointed as Vice President.
