I suppose you call them either a Jew or a Muslim since Judaism and Islam are separate religions and you cannot be both. There is also no specific term for a Jew who converted to Islam or a Muslim who converted to Judaism in either religion. (Apostate is general term for abandoning your faith.)

It depends on the instance. The Nazis made the claim that anyone who had one Jewish grandparent was Jewish. The Israelis have slightly different criteria. The Spanish Inquisition made the claim that someone was a Jew if that person claimed to be a Jew or if they failed to be a "proper" Christian.
There are two ways that a person can be Jewish - either by being born to a Jewish mother (or, in some modern communities, to a Jewish father - though this is not strictly in keeping with Jewish law) or by conversion. Jesus was born to Mary, who was Jewish (as was her husband Joseph) and so he was a Jew. Since not everyone has a Jewish mother and not everyone has converted to Judaism, not everybody is Jewish.
