Mainly because they do have bad teeth...I know, I am a Brit. The cause is the National Health Service which is supposed to cover dental work, but the payments to dentists are so mean that only the worst dentists will work for it, and thay have typically two year waiting lists. Finding a good dentist who employs a hygenist is difficult, and they are EXPENSIVE, so lots of people just do without.
