its to prevent the mother and her baby from getting infections.. some patients might go into labor and give birth in an unsterile environment, which can infect the baby and the mother later on.. some patients might think they dont need it, but still its recommended to take it just to be safe. taking it wont cause harm, other than the needle pain.. if you are pregnant, consult your health care provider about this..

Neonatal tetanus (NT) remains a major public health problem in developing countries [1--4]. The World Health Organization's (WHO) implementation of the Neonatal Tetanus Elimination Program in 1989 was a major step toward eliminating this disease [5], however, even though a safe and highly effective vaccine is available, NT still causes an estimated 250,000 deaths annually worldwide.

The most effective ways to prevent NT are to vaccinate reproductive age women with TT at the time of pregnancy, and to ensure sterile conditions at delivery. This program required that all non-immunized women be vaccinated with two 1-ml doses of aluminum-absorbed TT during their first pregnancy, and with one dose during each subsequent pregnancy.
