This could largely depend on what the exact problem with the image is. If an image is corrupted or made improperly, it won't work correctly later. Also, some end users make their images incorrectly. If you've made an ISO file and put it on a disc to put into multiple computers, there is a slight possibility that the optical drives of some of the computers cannot read that brand of disc correctly. This does happen sometimes, as not all optical drives are guaranteed to be compatible with all brands of disc. Also, it is important to make sure a compatibility check is done for the systems the OS is going on. If make an image for a 64-bit OS and try to put it on a machine that can only run 32-bit you will have a problem.
