All internal organs are sensitive, the testicles, the liver, the heart, the lungs, etc. The reason why testicles seem to be particularly sensitive is that they are not inside the torso, and they do not have the same protection of layers of muscle and bone, that other internal organs have, they have only a thin layer of skin, the scrotum, to protect them from whatever shocks and impacts they may experience. So the real question is not why testicles are so tender, but why are they positioned outside the torso. And the reason for that it, they need to be slightly cooler than other organs do. They are outside of the torso so that they can cool off. This is part of what they do, it helps with the production of sperm cells.
