It is perfectly normal for your wheel to not want to come free after pulling the lug nuts off. two simple solutions are using a rubber mallet to hit the sidewall of the tire to help bust it free, or what i personally do is "donkey kick" the side wall of the top of the tire... either way will bust it free.
Once you have undone and removed the wheel nuts and jacked the wheel clear of the ground it should come straight off, if it doesn't it may need to be pursuaded with a little brut force as many miles of motoring, brake dust and rust will have bonded the wheel to the hub, ensure the car is secure and on level ground before you 'bully' the wheel off.

With car on ground and all wheel nuts tight. Loosen all wheel nuts 3 complete turns. Drive car forward and back about 10 feet. Jack up and remove wheel nuts and wheel.
Jack up the car, and remove the 4 lug nuts holding the wheel on. (it maybe easier to loosen the nuts before jacking up the car).

To change the a wheel assembly on a car: 1. Loosen the lug nuts. 2. Raise the car as required and put on a stand. 3. Finish taking off the lug nuts. 4. Remove the wheel and put the replacement wheel in place. 5. Put the lug nuts back on, remove the stand and lower the car. 6. Tighten the lug nuts.
5 lug nuts is common in 3/4" size.
On a 2000 Lincoln Town Car : ( there are 5 lug nuts per wheel , total of 20 lug nuts )
