The plasma and red blood cells of a person's blood account fully for both A and B antigens and antibodies in some combination, and if both A antigens and A antibodies are present in the same mixture, they react negatively to each other. Blood transfusions only deal with the red blood cells, which hold the antigens, but O type blood has both antibodies in the plasma, which will react negatively to any red blood cells with antigens.

People with blood type A have so-called "antigens" on the surface of their blood cells. This is fine for them, because their immune systems have developed not to react to these antigens.

People with blood type O do not have these antigens on their cells, so if A-type blood is given to them, their immune systems will attack it as a foreign invader. This causes the blood to clot inside them, which can cause damage and death.

Blood type O has both A and B antibodies in the bloodstream.

Mixing any blood with A and/or B antigens in its cells (the blood type describes which are present) in a blood type O body will result in the antibodies in the receiver's blood reacting to the antigens in the donor blood cells, causing the cells to dangerously clump.

On the other hand, since blood type O has no antigens present in its blood cells against which to react, this blood can be safely donated to anyone else.

NOTE: This description is dependent on the donor's and receiver's Rh also being compatible.

Type O blood, found in approximately 40-45% of the world's population, does not have the terminal sugars on red cell membranes that determine type A, B or AB blood groups. In the ABO system, blood types follow Landsteiner's Rule: An individual will produce naturally occurring antibodies directed against non-self ABO antigens. Since type O RBC's do not have A or B sugars, "O" red cells may be transfused to generally any recipient (respective for Rh). Type O recipients, however, have strongly reactive anti-A, anti-B and anti-AB IgG and IgM antibodies (unique to type O individuals) in their plasma. If incompatible red cells are transfused to an "O" recipient, an acute hemolytic transfusion reaction may occur, potentially leading to renal failure and death.
