The basic assumption made in the question is incorrect. Everyone did not and does not wish to be anorexic.
It came to prominence because many people had a poor self image and believed much of the hype made by fashion trends. Exacerbated by companies using very underweight models. There were several minor celebrities with the illness that added to the problem by people believing they were good role models.
