*MAJOR SPOILER ALERT*
Hollyleaf as a kit an apprentice, and most of her warrior life, lived for the warrior code. When she found out that Leafpool was her actual mother and Crowfeather was her actual father, she went crazy. She announced this at the gathering and left going right back to camp. Leafpool caught up with her and Hollyleaf admitted to killing Ashfur. Then Holly attempted to kill Leafpool with deathberries but failed. Later, she ran away from her pursuing brothers and ran into the tunnels. They collapsed from a mudslide and Hollyleaf was said to be dead. In Sigh of the moon, Jayfeather and Lionblaze explore the tunnels and found where the rocks collapsed over her but only found a tuft of her fur. She could still be dead due to life as a rouge but it is rumored that she comes back with kits in the forgotten warrior.

Hollyleaf's mental breakdown was triggered by the discovery of her true parentage and the guilt she felt for keeping the secret. She struggled with the conflicting beliefs of the warrior code and her loyalty to her family, which eventually led to her unraveling.

Hollyleaf she goes crazy and kills Ashfur. she runs into a tunnel and it falls in on her and kills her. she wasn't one of the three. P.S. She isn't killed yet, find out in Hollyleaf's story and I think Hollyleaf reveals himself in The Forgotten Warrior.

Because Hollyleaf loved the warrior code, and when she found out that her birth was against the code she went crazy. And Hollylead never exiled her mother, Leafpool was never exiled from ThunderClan.

How does Hollyleaf know that Crowfeather is her dad?
At the end of Sunrise, Yellowfang brings Jayfeather a crow's feather in a dream, and tells him that StarClan were wrong to lie to him about who he really was and where he came from. Jayfeather figures out that the crow's feather means that Crowfeather is his dad, and he tells Lionblaze and Hollyleaf. Hollyleaf is extremely upset, believing that her birth crushed the warrior code. At the Gathering, she reveals to everyone that Leafpool and Crowfeather are really her parents, not Brambleclaw and Squirrelfight. The Gathering breaks into pandemonium, and everyone is distressed and upset, especially ThunderClan. Hollyleaf realizes that what she did just brought unwanted attention to herself and her clan. Leafpool leaves the Gathering, and returns to ThunderClan's medicine den. Hollyleaf finds her, and tries to kill her with deathberries, but figuring Leafpool living would make her suffer more, she lets her go. Going crazy, Hollyleaf races to the old tunnels and throws herself into the underground river. The cave closes in on her, and she dies.

Hollyleaf kills Ashfur in self-defense to protect herself and her brothers (Lionblaze and Jayfeather). She pushes him into the stream where he had previously chased her, drowning him.
