whilst the answer above has some truth to it, it does lack the fact that America is composed by many countries not just the US. and whilst I am searching for the answer to this question myself I do have one small theory.
It appears to me that the US became the spot known to immigrants and so whenever the british spoke of someone pertaining to america they were generally reffering to people from the US. Like many words in the english language it simply evolved to have that one meaning. This of course its just a theory, Im still looking for the answer.

Of course. African Americans are born in the US, so they are automatically citizens. To get more technical, if an African came to America and became naturalized, s/he'd be a US citizen.
