The bus driver has no way of knowing who plans to get off at the next stop. It is logical that people who are planning to get off will stand near the door so that they can easily do so. If someone who actually isn't planning to get off also chooses to stand near the door and to get in the way of people who are trying to get off, that person looks just the same as someone who is planning to get off. You can't tell.
Also bear in mind that just driving a bus is a complicated activity, which involves watching the other traffic closely to avoid collision, and so forth. A bus driver has only so much time and attention available to direct the passengers.
Most people probably can't stand on there toes without something being there to support them(other than the floor). I do know some people that can stand on their toes but it takes lots of practice and if you don't do it correctly, it can result in injury.

Some people have designated drivers. Or just go to have like 1 beer. Or just soda. Or work there. So those people could drive.
In order to handle problem people, you would need to have lots of patience. Listen to what they have to say and ensure that you explain to them your stand without arguing with them or making them feel that they are wrong.
It is impossible to find the exact demographic of truck drivers to car drivers in Kentucky. However Kentucky does experience moderately cold winters and has lots of farm land so chances are high that there are many pick up truck drivers in Kentucky.
