There are many reasons. I personally am underweight.
#1 Beauty is in the eye of the beholder. Maybe they find it beautiful.
#2 Their features (genetic appearance) makes it so they look much better underweight. Like me for example. i have delicate features. My face looks sad and fragile. So I look better when my body compliments it.
#3 Body dysmorphia. This condition makes it so the person looks in the mirror and hallucinates minor to nonexistant flaws as being severe. They may think being a healthy weight is fat because they hallucinate.
#4 Bullying. Bullies can make someone fell hideous when they are in fact gorgeous.
