Probably because of the strained relationship since 9/11. But western airlines don't just avoid Pakistan. Most western airlines avoid India as well (only United and Delta fly to India, United from Newark to Mumbai and New Delhi, and Delta from Amsterdam to Mumbai), but alot of western airlines used to fly to India (American Airlines from Chicago O'Hare to New Delhi and Delta Airlines Atlanta and New York JFK to Mumbai).

Although Pakistan International Airlines flies New York JFK to Karachi and Lahore through Manchester, Air India operates service from Ahmedabad and Mumbai to Newark nonstop, as well as New Delhi to New York JFK and Chicago O'Hare nonstop. Jet Airways also flies Newark to Mumbai through Brussels, Belgium. Biman Bangladesh also offered Dhaka to New York JFK nonstop.

The only hope of reaching Pakistan from the USA is through Europe, East/Southeast Asia, the Middle East, or India.

Why don't any western airlines fly to Pakistan?

Due to Americans in the Americas afraid of Pakistanis due to the New York attack of 9/11, no western airlines fly to Pakistan, and Pakistan International Airlines, the sole operator of flights from Pakistan to the Americas (specifically New York and Toronto) are not non-stop because of supposedly terrible security. Pakistan is not jealous, as Bangladesh does not have ANY flights to the USA, although Biman Bangladesh Airways usde to have non-stop flights to New York. United Airlines, Jet Airways & Air India are the sole operators for India flights, with nonstop services from Chicago and New York to New Delhi (Air India) and from Newark to Mumbai (Air India, Jet Airways, United) and Delhi (United). American Airlines also flew from Chicago to New Delhi but that was cancelled in 2012.
