There are basically 4 reasons this happens: 1) The heat A/C head control unit has gone bad. 2) The solenoid bank behind the ash tray has gone bad. (has many colored hoses connected to it) 3) There is a vacuum leak / no vacuum to the controls (yellow hose leak) 4) The vacuum actuator valve is broke. (The valve is on the driver side, where your feet are, up in the dash on the right hand side. Has a yellow and brown hose going to it).

You have to completely remove the dash to get access to the heater core.
