Because the ink must be removed from you skin, which in a sense has had ink injected into it to dye it. The process isn't simple because it must take out all of the color and ink engraved into your body.
It really depends on where on your body you get your tattoo

Dermabrasion for tattoo removal can hurt very badly. It depends on where the tattoo was and the size of the tattoo removed. Dermbrasion on the ankle can hurt very much due to normal walking or normal movement. Most of the time it requires dressing changes which are daily and can be painful until the skin heals. It is over all better than a bad tattoo and does not scar or rather I have not had scarring

Yes, getting a tattoo does hurt, some sessions hurt more then others. The important thing to remember is that after the pain and the expense has gone, all you are left with is the tattoo, so don't wimp out on size or location because you are worried about it hurting. It's only for a couple of hours. And remember that you can get it removed, although the area of skin where you got the tattoo will never look the same again. It just feels like rubberbands snaping on you hard.
It feels like little needles going into your skin. The pain depends on where you get the tattoo. Areas that are more bony hurt more than fleshy areas.
