Since I can't see how you hold a pen/pencil I am going to assume you are holding them wrong. I have had a number of students complain about the same problem and they are holding the pen/pencil too close to the tip. In doing so the fingers begin to cramp and they are holding the pen/pencil too tightly as well. To help you hold a pen/pencil higher on the body you can get a rubber grip that will position your hand in the right angle/area. This will help you. You want your fingers further up the pen/pencil rather than down on the point. If you rest the pencil/pen on the forefinger with the index finger as a guide this should help you. It may feel funny at first, but it works. When you are writing, now and then stop and stretch your fingers while you think of the next thing you are going to write about.
