To be more specific, North Korea is feared because they have a military with powerful enough weapons to pose a threat to any nation on the planet, and are desperate enough to be unpredictable.
Their only real ally of significance is China, and their indoctrination of their citizens can only last so long, meaning they're backed into a bit of a corner and no one knows what they'll do.
