Either the belt is worn down, has some oil on it, or the belt is under-tensioned. If the V-Belt is glacy or cracked on the side wall, it is run down. Replace it. Else, If the belt is oily, figure out why, fix that problem, clean oil from pulleys, replace belt. Else the belt is undertensioned. If none of the above, then the belt may be the wrong dimension.
Disconnect the battery, remove all wires from the alternator, slacken the belt tensioner, slip the belt off of the alternator pulley, unbolt the alternator, and remove the alternator. Installation is the opposite of removal.
Disconnect negative battery terminal. Loosen the serpentine belt - loosen the tensioner. Disconnect the alternator connections at the terminals. Slip the serpentine belt off the alternator pulley and pull the alternator out. Install is reverse.

If you can, keep the belt on all of the other pulleys as best as possible. It's a pain to get it back on. Remove alternator, then put everything back together with the new alternator.
It can be several things, either by themselves, or in combination. 1) the belt has dried up and isn't as grippy any more. This causes it to slip and squeal. There are sprays available to restore grip on belts which might help. 2) the belt has stretched a little, which causes it to slip and squeal. Retension, usually by adjusting the alternator position and it should be OK. 3) one of the auxilliaries(alternator, power steering pump, water pump) is about to seize. If it's harder to rotate the belt will slip and squeal.

Loosen the alternator tensioner bolt. The alternator belt will become loose. Remove the alternator belt. Put the new alternator belt on and reverse the process.
Loose or worn fan belt would cause alternator not to spin fast enough to charge battery. Worn bearing in alternator could be causing it to seize and causing belt to slip
Yes, I know of no engine where the alternator is driven by the cam belt. The alternator is driven by a drive belt or serpentine belt.
