1. The engine is getting too hot. That is what the fan is for, to cool it down.
2. An electrical fault causing the fan to be on when not needed.
As for the causes of 1 & 2...
1. Failed head gasket, stuck thermostat, failed water-pump, blocked pipes, low coolant level, faulty radiator... And many others.
2. Stuck fan-switch, stuck relay, damaged wiring, Someone modified the car to make the fan on all the time... And others.

While your car is running the engine is heating up. While your driving you have air being forced over the radiator cooling it off. When you are sitting still you don't have that air so your engine is heating up even more so the cooling fan comes on to help cool you engine off. Via cooling the radiator.
