The blend doors are moved by electric motors on a 1998 Voyager. Vacuum is not used.
To replace a blend door on a 1999 Voyager, you will begin by checking which side of the vehicle is not receiving heat. Then, remove the glove box door by squeezing the inside of the door toward the center of the door to access the blend door.
The blend door actuator is under the glove compartment near the blower motor. You will see a wire attached to it and will need a small nut driver to remove it.
Can't say for positive sure, but I would expect the Freestar/Monterey blend door actuator motor to be much like the Windstar. Common failure, easy replacement - about $40 See "Related Questions" below for links to the Windstar related blend door discussions
behind clove box it is inclosed in the black air duct system there is an acuater that controls opening and closing of the door the blend door controls hot air and cold air flow
The "blend" doors have broken in the dash. You will need to have the entire dash removed and the blend door replaced. You should also have the recuirculation door replaced because it is prone to breaking too.
Could be, low on coolant, a stuck open thermostat, a plugged up heater core, a faulty temp blend door mechanism,
most likely a vacuum leak or faulty control on the dash

The diverter, or blend, door, is not working properly, The door is either feeding the heat through the defroster, or closing the heat completely off.
