An automatic gear would stick in 1st if there is an obstruction. Gears can sometimes break off and cause motion to stop entirely.
Normally you have to listen to the RPM rate to know when to change into ANY gear on a stick-shift/manual transmission car. Changing between fourth and fifth gear isn't any different from changing between, say, 3rd and 4th. I wouldn't bother keeping too much of an eye on the RPM meter, because if you listen closely the engine will tell you when it is necessary to change the gear.
The gear stick has a diagram of where to move the stick to engage a certain gear. For example, if Gear 1 is in the top left corner, you must move the stick to the fair left, then forwards to engage first gear. Oh, and you have to push down the clutch each time you change gear.

You need to lift the collar on the gear stick to engage reverse gear
I posted this question, as it turns out the auto transmission just needed a good service, it hasn't jumped out of gear since the service, and is even changing gear smoother.

To remove the gear stick from a Nissan Qashqai J10, first, ensure the vehicle is in neutral and disconnect the battery for safety. Carefully pry off the trim surrounding the gear stick, usually held by clips or screws. Once exposed, detach any retaining screws or clips securing the gear stick assembly, then pull the gear stick upwards to remove it. Make sure to disconnect any electrical connectors if present before complete removal.
If it is done right without grinding the gears, nothing. You need to take the load off the gearbox, by lifting of the gas slightly, when changing up. Pause in neutral before easing the next gear in. Changing down is harder to do smoothly. Lift off the gas, pause in neutral, increase the revs to what you think it should be, for the speed in the next gear. Ease the stick into the next gear down.
