your radiater could be bad or caked up with mud or dirt. flush out the radiater and put new coolent but before you do that make sure your radiater is not bad. if its bad than just replace it.

You rear heat will only work if your front heat is on heat and feet. If it is on vent at the front of the van it will not heat the rear of the van. Not sure why but that is how ford designed it.

The unit is over the back door, The pipes/tubes for both A/C and heat run from the front of the van along the underside of the body on the frame . The A/C evaporator for the rear unit is located at the rear of the back drivers side wheel . If it's a conversion Van there will be a simple piece of carpet over it.
