Yes, you can have spotting without a period while using an IUD.

Take a pregnancy test to find out. Back pain is not the chief symptom of pregnancy, and brown spotting can be normal with the IUD.

If its a mirena, it is pretty common to have spotting. I spotted for 6 months straight and finally had it taken out because I could not deal with the constant spotting.
