There are several reasons. One is that while he campaigned as a moderate governor and promised to run the state that way, within two years, he had decided to run for president; suddenly, he was not in Massachusetts very much, and instead of governing, he was going around the country making fun of so-called "Massachusetts liberals," many of whom had voted for him and were not amused to be punchlines in his jokes. Another reason is that while he did not raise taxes, he instead put in many new fees on a wide range of government services, and the excessive number (and dollar amount) of the fees irritated many voters. Thus, while Massachusetts voters were pleased about the health care law (which is still popular), they felt that he presented himself one way and then dramatically changed.

Actually yes, but not together and not at the same time. When Mitt Romney was Governor of Massachusetts from 2003-2007, he did in fact sign into law a health care bill that is still widely popular in Massachusetts. Barack Obama's version, put forward in 2009-2010, is similar to Mr. Romney's, and incorporates both Republican and Democratic ideas.
