Burrs at the strike point of a chisel can break off and because of the impact speed and force of the hammer hitting it will cause the burr to travel both high speed and with significant force.
Most hospitals having accidents referred to them from this type of accident see server eye injuries, both for the person holding the chisel AND for people in "firing range".
That is why eye protection is so vital. Further to this are puncture wounds caused by these high speed, jagged missiles embedding themselves in the skin.
The other, but not so common injury is where the hammer strikes the burr more that the actual chisels strike point, the burr collapses and the hammer can carry through causing the hand to hit.
