Possibly a bad heater core. Could be simple stuff though. Have you checked your coolant level? Sometimes those "doors" in the dashboard for the heat and a/c are not opening and closing properly.

Commercial Thermoses basically use two bottles (a smaller one secured inside a larger one) and attach them at the opening. They then pump the air out of the space between the bottles, creating a vacuum. Because they require matter to happen, this essentially eliminates the possibility of conduction and convection in the space between the bottles. With the primary methods of heat transference eliminated (except at the opening) only radiation can cause change of temperature. If you're looking to make one yourself, I'm guessing you don't have the equipment to pump a vacuum. Air, foam, water, and human skin are all excellent insulators that you could use to fill the space between the bottles.
