Changing leaders is almost always required because culture is very difficult to change except from the top. A new leader can change a culture. They can bring a new vision of the future, new rules, new rewards structure, and new behavioral expectations which will quickly roll down the organization.
It is possible, in theory, to change culture without changing the top leader but it would take some incredible personal change for a leader to pull that off. The only example I can think of is Chairman Mao's Cultural Revolution - and that didn't work out very well for China.
If a culture does change bottom-up (Arab Spring is an example) old leaders will not stay in power and new leaders will attempt to ensure the change is institutionalized.
