Bad plug wires, there was a recall on the 1998 Firebird 3.8L. GM would only replace each
one as they failed. The check engine light should come on, check with ODB II which should indicate the cylinder or cylinders no. that have bad wires.
Cylinder missfire perhaps ? Get it scanned.
heat shield might have came lose if it rattles alot

It is possible that the shaking in a Pontiac Montana at 45 MPH is the result of failing bearings or tie rod ends.
If the shaking is specific to acceleration and the shaking is very rythmic (not random) it is most likely a fault in the ignition system. Usually a tune up or ignition coil replacement will solve this problem.

Check the tire balance. Also check the tie rod ends and ball joints for wear. These are the most common places to start.
