Spoiler!! As soon as you return the mask to the tomb in Escalera, you'll be transported back to his house where he finds out his family is ok. It then goes to a cut-scene where Seth steals the mask and then to another scene of John's grave and him coming out of the ground. You'll still have all your weapons and a deed to the horse "Death".

It's seperate to the original Red Dead Redemption. You can tell straight away as John Marston is still alive.
