The rosary is probably the most basic prayer that a Catholic learns to meditate with, and not only begins, but carries it with them throughout life. In every one of the appearances of Our Lady (Jesus' mother) she always requested the rosary to be said. Putting the rosary in the hands of the deceased is both a reminder to pray for them, and a plea to Our Blessed Lady to deliver them from purgatory.

It's the other way around, the physical rosary was put together the way it was so that you would be able to see where you are in your praying of the Rosary.

Is it appropriate to say the Rosary at the time of death?

It is not only appropriate but highly recommended. In fact, after the last rites, there is no devotion more highly recommended. Most Catholics at death, when too weak to pray the Rosary themselves, are usually surrounded by family who pray it together. When visiting the sick and dying it is very common to say a decade of the Rosary with them or for them and to give them a rosary if they do not have one. It is very common to bury a Catholic holding a Rosary; this is not for mere appearances - it is to be hoped the Rosary was actually used in life by the deceased or said for them and it is buried with them as a sign of their devotion and confidence to others in Mary's promise to work for the salvation of those devoted to her patronage.

Where do Catholics wear the rosary?

A rosary is not meant to be worn, outside of a religious habit. In other words, some religious brothers, sisters, monks, and nuns have a rosary hanging from their belt as part of their religious habit. Very occasionally, I have seen an older lady who walks to Church who does not have pockets or a handbag wear her rosary around her neck, but this is rare. Young people who have plenty of pockets and such should never have a rosary around their neck where it would show as this can be taken the wrong way by religious people who pray the rosary daily.

Which one of the rosaries do you say at a funeral?

In the "good old days" it was common for the mourners to pray the rosary in common for the repose of the soul of the deceased. For the past 25 years (+ -- Vat II) since my ordination, a "wake service" to include scripture readings, prayers for the deceased and mourners is most common. From my experience, when a rosary is requested few, if any, of the mourners recite it.
