It depends on the manufacturer. Most rear wheel drive vehicle speedometer cables connect to a gear in the transmission "tailshaft". Front wheel drive vehicles may connect at another location on the transaxle or they may use an electronic Vehicle Speed Sensor (VSS) that sends a digital signal to the speedometer, in which case there is no speedometer cable. Other vehicles, such as the old VW Beetle, used a speedometer cable that connected directly to the front wheel. Unfortunately, there is no way to list what method was used by all vehicles.

A speedometer that loses response time but gets up to the speed of the vehicle probably has a problem with a gear slipping inside the speedometer mechanism. This sometimes happens in older vehicles. The speedometer cable may have to be replaced to fix this problem.
It is possible. Most new vehicles have a sensor attached to the transmission that tell the speedometer how fast the vehicle is traveling. I suggest checking, or having the sensor checked first.
Federal Motor Carrier Safety Regulation 393.82 states that "each bus, truck and truck-tractor must be equipped with a speedometer indicating speed in miles per hour and/or kilometers per hour. I am from Canada, and law here for commercial vehicles states that if it had it when it left the factory, it must be present and in working order. This would include the speedometer. If your vehicle had a speedometer when it was new, I am assuming U.S. law requires that it be there and operational the same as in Canada. there is actually no law stating the speedo has to be working it also was never covered in a vehicle inspection
