One and only one answer..... INJECTOR cups. They have been known to crack and in that fuel pressure is much higher then coolant pressure it mixes that way and not the other. All the injectors will need to be removed and repressure check the cooling system and inspect all 8 cups for signs of leaks. If it was me i'd just do them all and be done with it. You need a few special tools and recommend a certified mechanic to do that repair not for you shade trees!! Good luck.
