If your console is software modded then it is due to either a bad copy, or the disc is scratched badly. When burning games you need to burn at 2.4x, make sure the layerbreak is set to 1913760, make sure you use dual layer DVDs, and use good quality media (VERBATIM DISCS ARE RECOMMENDED).
If your console has not been modded then it is due to scratched discs. Also the DVD drive laser lens could need replaced.
