The only thing that can be directly observed is a galaxy's apparent size (angular size). Since galaxies come in different sizes, this can only give a rough idea of the galaxy's distance from Earth.

How does One know that he is not alone is the universe?

Although it's impossible to know, one has to think about the vastness of our Universe and that it contains an estimated 100 to 200 billion galaxies - our galaxy is one among billions. If that doesn't blow your mind, our galaxy is 100,000 light years in distance - and a single light year equals about 6 trillion miles. Now, think about the distance between one galaxy to the next...trillions upon trillions of miles. Is it truly logical to think that we're the only forms of life in our Universe? I don't think so.

The Milky Way Galaxy is classed as a spiral galaxy as it's appearance is of a spiral with looser outer stars becoming denser the closer you get to the centre. As a picture is worth a thousand of my words, the the related link for a generated picture of our galaxy. Note: This is a computer generation of what we believe the galaxy looks like and not a real one (as we have not ventured past our own solar system, let alone the galaxy).
