There is no particular exemption for hospitals needing fire sprinklers. In fact, there are many fire hazards in hospitals that increase the need for fire protection.
A good design, however, may incorporate additional mechanisms to extend the time for release of water or for using a "dry pre-alert" system in certain areas. Even where there are fixed fire extinguishing systems (CO2, Halotron, etc), sprinklers are typically required "throughout" the facility.
