There are three main causes where an account could be compromised despite having a password. The first is that the password chosen is insecure, such as the name of a favorite animal, or is simply too small or simple. The second main cause is an unsecured administrator account, in which case the intruder could circumvent the protected user account through an administrator. Since most versions of Windows offer an unsecured administrator account by default, it is recommended that the account be renamed and password protected. The third most common cause is by using an insecure file system, such as FAT32. There are no file permissions beyond a simple single-user design, so files can be viewed by anyone with even guest privileges. The recommended course of action is to always use NTFS (only available on newer Window OSes). As a final note, passwords are meaningless if you have auto-login enabled for that account, if your computer is physically copied or stolen and you do not use BitLocker or another encryption algorithm, or the attacker can boot another operating system onto the computer, such as Linux, which negates NTFS file permissions (but encryption is still effective).

How does Unix protect passwords?

Passwords are protected in 2 essential ways: first, the file containing the password is only viewable by the administrator (root) account. Secondly, the password itself is hashed so that it appears to be encrypted, and is a one-way hash. This means there is no way to retrieve a hashed password (other than guessing). It has to be reset in that case.
