Astronauts were suits to protect them from the harsh conditions of outer space. In space, there is no air pressure, so without a suit to provide pressure the astronaut's blood would literally boil in a matter of seconds, causing a painful death. Direct exposure to sunlight too can be lethal, as well as exposure to the cold of space. A space suit insulates and protects against these things, and provides breathable air.

When astronauts travel in space they must wear special helmets?

The helmets are part of their space suits. They do not normally wear space suits unless they are going outside the spacecraft. Since there is no air outside and it can be very cold or hot. They need the suits to breathe and to protect them from the heat or cold and the vacuum of space which would kill them.
