Scout, Tenderfoot, Second Class and First Class ranks are designed for young men to learn the basics to take them through the program and live up to the goals and ideals of scouting. Much like a sponge, in these early ranks you suck up all the information, so as you go to higher ranks you can turn from taking in, to giving back. This is why the other ranks require service, and leadership. Merit badges are designed to expand that base of knowledge, and while they can be earned at any rank, are then applied when you reach Star and above.
There are no merit badge requirements for First Class. However, there are various individual first aid skills among the Tenderfoot, Second Class, and First Class requirements. First aid is an Eagle-required merit badge, though. See link for more info.

I think they just made a decision about how many ranks they wanted to have and what to call them. Originally, the ranks were Tenderfoot, Second Class and First Class. I would think they just thought that "tenderfoot" was a better description of an inexperienced scout than 3rd class. Within 2 years or so they added the ranks of Life, Star and Eagle, in that order. They changed the order of Life and Star partly so that the five points of the star could be said to represent the 5 merits badges required to earn it. They later added a 6th merit badge and lost that sense but kept the new order. The Scout rank, which one attains simply for joining, was added some time after I was a scout in the late '70's to early '80's.
