When the cells were replicating, one of the cells messed up somewhere in replicating DNA or the transferring in DNA to both cells during mitosis (one cell dividing into two cells). The part of the DNA that took care of the eye that your nephew was deprived of got caught up in a mix somewhere along the cell division process and is now eye-deprived. There could also be other reasons, but this is just one of them.
