Sounds like snuffles to me. You need to get the rabbit to a vet.
You rabbit has an infection, called "snuffles" or "sniffles". It's most likely caused by Pasteurella. You should seek veterinary treatment immediately, because your rabbit needs antibiotics to cure its infection.
Please note: You should be extremely careful what antibiotics your vet prescribes, because drugs in the -cillin family (Amoxicillin, Ampicillin) are toxic to rabbits. Your vet will probably want to prescribe Baytril or another rabbit-safe antibiotic for treatement.
