Since it seems to be related to whether the O/D is engaged or not, it sounds like a transmission problem. :-( However, before going that route - it MIGHT also be related to a bad motor or transmission mounting block (loose or broken), or perhaps, even an out of balance, or out-of-round tire. These possibilities are relatively simple and inexpensive to cceck out and eliminate. Once you get into the trnsmission it's gonna get expensive REAL quick.
