The main challenge in treating schizophrenia is keeping the patient on medication. Antipsychotics are unpleasant; the patient may feel as if everthing has lost its meaning, life has lost its color... I've heard someone describe it as if your whole body is covered with an inch of vaseline - you can't really feel anything. Most of the time, schizophrenia is much more pleasureable than being under antipsychotics. Because of this, a lot of patients may find ways to evade taking their medication.
Although it's difficult, it is possible sometimes to treat schizophrenia through hard work and sheer will. John Nash, a famous mathematician and schizophrenic, used rationality and logic to convince himself that his hallucinations weren't real. However, this isn't always possible.
