Possibly because the light stayed on for an extended period of time. Other, reasons may be because of an electrical problem in the wiring of the vehicle, causing the lights to malfunction, causing a rapid, continuous switch from on to off in a quick period of time. If you press the brake pedal and let go numerous times in a short period of time, that may also cause the taillights to heat up. Another reason may be that the bulb was not the proper type. Also, if it was the proper type, there is always the possibility that it was not correctly installed.

Melting plastic is tricky because you can easily char or burn it and can be dangerous because of the fumes released when the plastic melts. Also, different materials melt at different temperatures which can also make it difficult. If you have access to a pottery kiln, you could try putting the plastic in a kiln-safe container and melting it that way. However, it is unknown what will happen if you do so. Either way, I would not suggest melting plastic at home, just re-use it for something else or recycle it.

Open the back of your Rav4, on both sides of the back from the inside, there will be a plastic cover over where the brake light would be, gently take that off and you can see the brake lights from there. Just twist the harness that the bulb is in and the bulb should just pop out. Good luck.
I would suggest looking for a mattress protector that does not have a plastic surface. Memory foams tend to collect heat and you wouldn't' want the plastic to trap the heat. Look for a breathable mattress cover.
I'm unsure of if you mean individually or the entire light. The clear plastic covers seal can be broken off, and then the internals modified. You then would have to use clear sealant to replace the cover. If you mean the entire light, definetly possible.
Sounds like a tail light fuse is out. This fuse is often on the same cicuit as the dash lights so that when the tail light fuse blows out the driver is made aware of it by not being able to see the dash lights. The fuse panel is on the left kick panel near the driver's left leg and is accessed by opening a small plastic cover.
