the bottle is defective and has a hairline crack! Mazda knows this but wont cover it unless you have a receipt or are in the warrenty period!

Check radiator hose, leaking water pump, look for hole in radiator, leaking coolant bottle or leaking seals.
Bad head gasket? Bad thermostat? Defective radiator cap?

A leaking gasket, a leaking filter, or a leaking drain plug....
Your coolant pump is behined the fan, you will have to replace the water pump.
