When it came time to kill every one he could not kill Sasuke, even though he was supposed to, because he was his little brother. Itachi made sure no one told Sasuke about the mission so he could live being proud of his clan. He wanted to die by the hands of Sasuke so his little brother could be regarded as a hero. Thinking back, Sasuke remembers that, on the night of the massacre, when he ran after Itachi, he saw him crying. But by the time Sasuke finds all this out, he had already killed Itachi. He is always on his way of dying. He is very near dying in naruto manga 468. But he manages to hang on. The author actually said this in the back of one of his books: That's just why I can get near to Sasuke's death and Kakashi's death, but if I actually kill them for good, I will be killed by a mob of fangirls in my sleep.
