Marijuana cannot be prescribed anywhere in the United State as prescribing a level one narcotic will cause them to lose their license. Level one narcotics are show to have no medicinal value.
No, there has not been any records of someone dying from marijuana. And if marijuana was fatal, I highly doubt doctors would prescribe it to cancer patients.
If its needed for medical use surely your physician can answer that question. Not all physicians prescribe that type of therapy so most clinics don't carry it. I would imagine those that do only carry it in small amounts do to safety reasons.

After you have done this the doctor will decide if this treatment is best for you and finally prescribe it if they decide it is necessary. I believe that in most states where the use is allowed that there are certain places you need to go, and generally they will give you an id card saying you are allowed to posses the marijuana for medical reasons along with you marijuana.
