Well if she knows sh's your girlfriend, then she's expecting you to ask her out. It doesn't even have to be too a fancy place. You could go to the movies, go for a walk at the beach, go to the park and just chill out or even take her to a place where you go by your self. Like to the top of a ill or something. You could have picnic and jus talk. Mainly just ask her, how? Call her, or catch her when she's alone. Tell her you'd like to hang out with her. I'd say 3 or 4 dates is good, though it's some what lame to some one you are going on dates with to date you, if you guys are going on dates and kissing, handholding, whatever there is little need to set a clean date or moment when you became boyfriend/girlfriend, just let it happen and grow naturally
