you have to remove the glovebox entirely. when you do there will be a fairly large plastic box exposed. on that box you'll see a small door or hatch (approx 2"x8") there may be tabs to push in or a little tab sticking out that you can pull on to remove the door. inside is a small drawer that holds the filter. replace it the same way it came out.

My 1998 E320 did this and it was the fuel pump. The dealer also replaced the filter and it was $1,100.
On my 2003 4Runner and my 2008 Tundra it's directly behind the glove box. Remove the glove box and you will see a long narrow door. The filter is behind it. Instructions came with my new cabin filter when I bought it from a auto store. ($28) Yours may be in the same place.

I believe the cabin air filter is located in the glove compartment. maybe behind that, You will have the check with your local parts store or with a Toyota dealer for the filter.
